目的 探讨我国大病保险政策内部结构的均衡性及各维度间的匹配性,为我国大病保障制度的编制与优化提供理论依据。方法 基于政策工具、政策目标、政策力度三维分析框架,采用内容分析法和政策文献量化分析法,对我国2012—2022年颁布的大病保险相关政策文件文本进行多维分类和交叉分析。结果 政策工具维度中,政策工具运用失衡,内部结构差异较大;政策目标维度中,政策目标有所侧重,但规范监管考核仍需进一步强化,且整体协同性不足;政策力度维度中,政策力度围绕着总体平均效力上下波动幅度不大,呈现波浪式前进的趋势。结论 目前,我国大病保险政策在组织建设和人才培养方面的使用频率明显过低,规范监管考核的政策目标占比也较低,且多部门共治支撑各政策目标有效实现的格局尚未普遍形成,政策整体协同性有待进一步提高。建议统筹协调大病保险政策工具的运用,调整内部结构并完善政策目标的结构,加强监管考核的规范力度,以提高政策体系的协调性,优化实施效果。

Objective To explore the balance of the internal structure and the matching between various dimensions of China's serious disease insurance policy, so as to provide theoretical basis for formulating and optimizing China's serious disease insurance system. Methods Multidimensional classification and cross analysis of policy document texts related to serious disease insurance issued from 2012 to 2022 in China were performed by employing content analysis method and quantitative analysis of policy literature based on the three⁃dimensional analysis framework of policy tool, policy objectives and policy intensity. Results In the dimension of policy tool, the application of policy tool was unbalanced and the internal structure was quite different. In the dimension of policy objectives, the policy objectives were focused to a certain extent, but the standardized supervision and evaluation still needed to be further strengthened, and the overall synergy was insufficient. In the dimension of policy intensity, the policy intensity fluctuated little around the overall average effectiveness, showing a wave⁃like forward trend. Conclusion Currently, the using frequency of serious disease insurance policy in organization construction and talent cultivation in China is obviously too low, and the proportion of policy objectives of standardizing supervision and evaluation is also relatively low; furthermore, the pattern of the effective realization of various policy objectives supported by multi⁃departmental joint governance has not yet been widely established, and the overall synergy of policies needs to be further improved. It is recommended to coordinate the use of serious disease insurance policy tools, adjust the internal structure and improve the structure of policy objectives, and strengthen the standardization of supervision and evaluation, so as to improve the coordination of the policy system and optimize the implementation effect.
