血必净注射液对离体空跳猪心缺血再灌注损伤的保护作用
Protective effect of Xuebijing Injection on ischemia⁃reperfusion injury in isolated beating pig hearts

目的 探讨血必净注射液对离体空跳猪心缺血再灌注损伤的保护作用。方法 将12头巴马小型猪随机分为血必净组和生理盐水组,每组6头。游离血必净组和生理盐水组小型猪心脏,将离体猪心连接体外膜肺氧合机管路(血必净组和生理盐水组分别在预充液中加入10 mL血必净注射液、10 mL 生理盐水),记录心脏恢复跳动时间(T0)及体外循环第8小时(T8)的心率、灌注压、灌注流量、血温、灌注转速。于T8取心肌组织进行HE染色;于T0、体外循环第2小时(T2)、体外循环第4小时(T4)、体外循环第6小时(T6)、T8时间点检测凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、D⁃二聚体,以及白细胞介素(IL)⁃6、IL⁃10、肿瘤坏死因子α(TNF⁃α)水平;于T0、T4、T8取心肌组织,采用Western blot和实时荧光定量PCR检测肿瘤蛋白p53、溶质载体家族7成员11(SLC7A11)、谷胱甘肽过氧化物酶4(GPX4)蛋白及mRNA表达水平。结果 两组小型猪离体心脏体外循环8 h时的心率、灌注压、灌注流量、血温、灌注转速及心脏复跳时间差异无统计学意义(P>0.05)。HE染色显示,T8时血必净组心肌损伤程度较生理盐水组轻。在T0时,血必净组和生理盐水组的APTT、TT比较,差异无统计学意义(P>0.05),体外循环期间两组肝素化效果良好;在T0、T2、T4时,血必净组的PT短于生理盐水组(P<0.05);在T4、T6、T8时,血必净组的FIB水平高于生理盐水组(P<0.05);在T2、T6时,血必净组的D⁃二聚体水平低于生理盐水组(P<0.05) 。在T2、T4、T6、T8时,血必净组的IL⁃6水平低于生理盐水组(P<0.05);在T4、T6、T8时,血必净组的TNF⁃α水平低于生理盐水组,IL⁃10水平高于生理盐水组(P<0.05)。在T4、T8时,血必净组的p53蛋白及mRNA表达水平低于生理盐水组,SLC7A11、GPX4蛋白及mRNA表达水平高于生理盐水组(P<0.05)。结论 在使用供体血离体灌注下,血必净注射液可改善离体空跳猪心的凝血功能,减轻炎症反应,抑制铁死亡相关氧化损伤,从而减轻离体空跳猪心的缺血再灌注损伤。

Objective To investigate the protective effect of Xuebijing Injection on ischemia⁃reperfusion injury in isolated beating pig hearts. Methods Twelve Bama miniature pigs were randomly divided into Xuebijing group or normal saline group, with 6 pigs in each group. The hearts of miniature pigs in the Xuebijing group and the normal saline group were excised, and the isolated pig hearts were connected to an extracorporeal membrane oxygenator circuit (10 mL of Xuebijing Injection and 10 mL of normal saline were added to the prime solution in the Xuebijing group and the normal saline group, respectively). The heart rate, perfusion pressure, perfusion flow rate, blood temperature, and perfusion speed at the heart recovery time (T0) and the 8th hour of cardiopulmonary bypass (T8) were recorded. Myocardial tissue was collected at T8 for HE staining. At T0, the 2nd hour (T2), 4th hour (T4), 6th hour (T6) of cardiopulmonary bypass, and T8, prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), fibrinogen (FIB), D⁃dimer, as well as levels of interleukin (IL)⁃6, IL⁃10, and tumor necrosis factor α (TNF⁃α) were measured. Myocardial tissue was collected at T0, T4, and T8, and the protein and mRNA expressions of tumor protein p53, solute carrier family 7 member 11 (SLC7A11), and glutathione peroxidase 4 (GPX4) were detected by Western blot and real⁃time fluorescence quantitative PCR. Results There was no statistically significant difference in heart rate, perfusion pressure, perfusion flow rate, blood temperature, perfusion speed, or heart recovery time during the 8⁃hour cardiopulmonary bypass in isolated pig hearts between the two groups (P>0.05). HE staining revealed that the degree of myocardial injury at T8 was milder in the Xuebijing group than in the normal saline group. At T0, there was no statistically significant difference in APTT and TT between the Xuebijing group and the normal saline group (P>0.05). During cardiopulmonary bypass, heparinization exhibited a favorable effect in both groups. At T0, T2, and T4, PT in the Xuebijing group was shorter than that in the normal saline group (P<0.05). At T4, T6, and T8, FIB level in the Xuebijing group was higher than that in the normal saline group (P<0.05). At T2 and T6, D⁃dimer level in the Xuebijing group was lower than that in the normal saline group (P<0.05). At T2, T4, T6, and T8, IL⁃6 level in the Xuebijing group was lower than that in the normal saline group (P<0.05). At T4, T6, and T8, TNF⁃α level in the Xuebijing group was lower than that in the normal saline group, while IL⁃10 level was higher than that in the normal saline group (P<0.05). At T4 and T8, the protein and mRNA expressions of p53 in the Xuebijing group were lower than those in the normal saline group, while the protein and mRNA expressions of SLC7A11 and GPX4 were higher than those in the normal saline group (P<0.05). Conclusion Under ex vivo perfusion with donor blood, Xuebijing Injection can improve coagulation function, reduce inflammatory response, and inhibit ferroptosis⁃related oxidative damage, thereby alleviating ischemia⁃reperfusion injury in isolated beating pig hearts.
