基因编辑技术与病毒载体的应用显著增强了脂肪干细胞(ADSCs)在恶劣微环境中的存活与功能,并实现了治疗基因的稳定表达,而表观遗传调控进一步精细调控了ADSCs的分化过程。这些工程化ADSCs不仅可作为细胞替代的来源,更能通过旁分泌与微环境重建发挥多重修复机制,在神经再生、骨与软骨修复、代谢性疾病、血管再生、平滑肌功能重建及退行性疾病等多个领域中展现出良好的治疗效果。本文系统综述了工程化ADSCs在多系统疾病治疗中的研究进展与应用潜力,旨在为未来个体化与精准化的再生医学策略提供理论依据。

The application of gene editing technology and viral vectors has significantly enhanced the survival and function of adipose⁃derived stem cells (ADSCs) in harsh microenvironment, and achieved stable expression of therapeutic genes. Epigenetic regulation further finely regulates the differentiation process of ADSCs. These engineered ADSCs can not only be used as a source of cell replacement, but also play a multiple repair mechanism through paracrine and microenvironment reconstruction, and have exhibited favorable therapeutic effects in many fields such as nerve regeneration, bone and cartilage repair, metabolic diseases, angiogenesis, smooth muscle function reconstruction, and degenerative diseases. This paper systematically reviews the research progress and application potential of engineered ADSCs in the treatment of multisystem diseases, aiming to provide a theoretical basis for future individualized and precise regenerative medicine strategies.
